How do hemorrhoids develop?

Hemorrhoids are enlarged veins that can appear inside your rectum and just outside the anus. All of us are born with hemorrhoid-tissue, but once it starts to swell or prolapse, the irritation starts.

There are several situations and behaviors that contribute to developing hemorrhoids:

  • Pregnancy: During pregnancy, the growing womb can put pressure on veins in your abdomen, which might lead to piles. Pushing during delivery might also cause or worsen piles.
  • Age: When you age, the tissue supporting the veins inside and around the anus weakens. This might lead to hemorrhoids.
  • Too much sitting down: Sitting on the toilet or behind a desk for long periods of time puts more pressure on your abdomen, which might cause veins to swell and result in piles.
  • Chronic constipation or diarrhea: These conditions can cause frequent pushing during bowel movements, which might cause hemorrhoids.
  • Genetics: Some people have a genetic predisposition to hemorrhoids.
  • Diet: A diet that lacks in fiber can cause constipation, which in turn can contribute to developing piles.

Can hemorrhoids be harmful?

Usually, piles are benign and not seriously harmful for your health. However, we do recommend treating them because they can cause discomfort and complications in the long run. If your symptoms are still persisting or worsening after two weeks, it would be wise to consult your general practitioner for a referral to a specialist.

Diagnosis

Your general practitioner will perform a brief examination to diagnose you with hemorrhoids. After, they will discuss your treatment options with you and refer you to a specialist.

If your general practitioner refers you to our Chirurgisch Expertise Centrum, we will inspect the affected area and your rectum with a proctoscope. This inspection is described by our clients as uncomfortable and they feel some pressure, but it is not at all painful. To cause you as little discomfort as possible, our surgeons are instructed to use the smallest proctoscope possible. After this brief inspection, no further examination is required for a diagnosis.

Treatment for hemorrhoids

There are different ways to treat your hemorrhoids. For example, the traditional (but rather outdated) treatment for hemorrhoids is placing a rubber band around the base of it. This cuts off blood circulation to the pile. This will be uncomfortable or painful for a couple of days, and it may cause some light bleeding.

We prefer a less painful method that is quicker and causes you the least discomfort. We inject a sclerosing agent directly into the piles. This causes the blood vessels inside the hemorrhoid to collapse, which in turn causes the lump to retract. The treatment only takes a few minutes, and afterward you can resume your daily activities right away.

When is treatment advised?

Most of the time, you can treat hemorrhoids at home. But if your symptoms linger or worsen over time, it is advisable that you seek professional treatment. For example, if you suffer from severe pain, bleeding or notable changes in your bowel movements. Contact your general practitioner if you believe professional care is necessary.

After determining the cause, we will start removing the piles right away by injection. First, we insert the smallest proctoscope possible to locate them. Chirurgisch Expertise Centrum provides medically insured care that is covered by the basic Dutch health insurance package and reimbursed by health insurers.

The basic health insurance defines the scope and content of insured care, which is the same for everyone. The rates charged by the Surgical Expertise Center are based on nationally determined market-standard rates. Agreements have been made with most health insurers, which means care is provided under contract. However, these agreements vary and depend on your specific policy and type of treatment. When scheduling an appointment, you will be informed about the available options, the type of treatment, and the reimbursement details.

If you are not insured in the Netherlands or if your care is not contracted, the self-pay (or “passant”) rate applies. For self-pay patients, the market-standard rate applies. This is a reasonable rate in the Netherlands for a specific treatment. Health insurers determine this rate based on what other healthcare providers typically charge for the same treatment. This market-standard rate may differ from the prices we have agreed upon with individual insurers.
